How to register your child and provide consent

Consent must be provided by a parent or guardian for their child/ren to receive vaccinations at school.

Most parents/guardians can register their dependants for a vaccination plan, and provide consent via a secure online consent form, which will automatically record their details in WA Health’s Vaccine Management System (VaccinateWA). If you are experiencing difficulty using or accessing the online form please email SBIP@health.wa.gov.au or call 9222 2486.

*If you're not able to access the online system, you can print a copy of the school-based immunisation program consent form (PDF 953KB) and return it to your child's school. Note: Details from the paper form will be entered into the VaccinateWA system so you may receive emails from VaccinateWA.

If your child attends a school on Cocos or Christmas Island or within the City of Joondalup, please use a paper form. If your dependant is a child under the care of the Department of Communities, please contact your dependant’s case worker to complete a form and return it to their school

Instructions for consenting online

  1. Access the online consent form (external site).
  2. Enter your child/dependant’s details.
  3. Enter your child/dependant’s school information.
  4. Enter your own details, as the parent/guardian of that child/dependant.
  5. Provide consent for the relevant vaccination plan selected.
  6. Acknowledge and sign the form.
  7. Submit the form. An email will be sent from VaccinateWA confirming your consent.

If you need to update any details, please submit the form again.
After immunisation

Following vaccination, students will receive:

  • a record of vaccine(s) received
  • instructions about care after vaccination.

Vaccines administered through the school-based immunisation program are recorded in the Australian Immunisation Register (AIR).

For more information on how to view your child’s record, please visit How to get immunisation history statements - Australian Immunisation Register (external site).

When a vaccine is missed at school (catch-up)

Catch-up is for young people who missed out on their vaccinations when they were first eligible.  All eligible individuals are recommended to receive the catch-up vaccine as soon as possible to be protected.

If they missed their vaccination at school, they can receive a catch-up vaccine from another immunisation provider such as at community immunisation clinics, or at participating GPs and Aboriginal Medical Services (the vaccine is free, but consultation fees may apply). This year, participating community pharmacists can offer adolescent vaccines at no cost. Consult your immunisation provider for more information.

Healthy young people aged up to 20 years old can catch-up if they have not previously received the dTpa or MenACWY vaccines.

Healthy young people who receive a single HPV dose before 26 years of age are now considered fully vaccinated and will not need further doses, with some exceptions.

Home-schooled students

Young people not attending school are still eligible to receive these vaccines in a non-school setting. Please see above section: “When a vaccine is missed at school”, for examples of alternative immunisation providers.
