First Older Adult Health Hub now open

WA’s first Older Adult Health Hub is now open in Cockburn.

This new centre brings multiple health services together in one location – including general practice, pathology, medical imaging, and community support services.

Located within the Cockburn Health and Community Facility, the hub aims to reduce avoidable hospital visits, helping older people stay healthier and independent in their own homes and communities.

The hub is staffed by a multidisciplinary team including geriatricians, nurses, allied health professionals, and care navigators.

Patients must have a GP referral to access the hub.
